2011 Income Tax Forms and Information

The Mount Prospect Public Library has a number of basic federal and Illinois income tax forms available for distribution. Additionally, assistance with printing/photocopying of other forms and publications can be obtained by request at the Reference Desk on the second floor. IRS Free File program provides free tax preparation software and electronic filing to taxpayers with an adjusted gross income of $57,000 or less. Some offer the program in Spanish and also offer state returns for free. For more information, go to www.irs.gov.

Seniors

Village of Mount Prospect
Mt. Prospect Human Services Dept. provides free income tax assistance for those over the age of 60, and low income households. Trained volunteer counselors will assist in preparing Federal and State tax forms (standard forms). Appointments are required. Call 847/870-5680 for more information. NOTE: A separate appointment must be scheduled for assistance with the Illinois Circuit Breaker application.

Wheeling Township
Free senior tax assistance will be provided by trained AARP tax counselors on

  • February 3, 10, 17, 24
  • March 2, 9, 16, 23, 30
  • April 13

Appointments are required. Appointment times are at 9, 10, and 11 a.m. For seniors age 60 and over. For more information, call 847/259-7730. All returns will be filed electronically.

Elk Grove Senior Center (225 E. Elk Grove Blvd):
Wednesdays and Fridays; appointment times at 8:30 am, 10:00 am and 11:30 am. February 1 through April 13, 2012. For more information and to schedule an appointment call 847/364-7224. For Seniors age 60 and older and low to moderate income (up to $60,000) taxpayers.

AARP Tax-Aide
is offering filing help to seniors.  Call for an appointment at the following locations: (Must be 55 years of age).

Tuesdays and Thursdays from 9:00 a.m. to 11:30 a.m. February 2 through April 14, 2012. Call 847/376-2788 for an appointment.

Low Income

CEDA Tax Preparation Assistance
CEDA Northwest, 1300 W. Northwest Hwy., Mt. Prospect, IL
Assistance will be available on Saturdays, February 4 through April 14, 2012.  Time:  9:00 am - 12:00 pm. This service is offered on a FIRST COME - FIRST SERVED BASIS--No appointments are needed or taken. Basic Tax Prep assistance (standard forms only) is offered for limited income (under $50,000) families, individuals, and to those with disabilities. All necessary documents must be brought to receive assistance including a picture ID, SSNs, last year’s tax return, W-2s, 1099s, proof of childcare expenses, and any additional income, deductions, or credits. Bring your checkbook to facilitate direct deposit of any refunds.  For more info, call 847/392-2332.

Free-For-All Federal 1040EZ: H&R Block will help anyone prepare and file the federal 1040 EZ form free-of-charge from now until Feb. 29, 2012. (There is a charge for filing the state form, but you are not required to file the state form to get the free federal filing.) There is an H&R Block office at 905 E. Rand Rd. (by Kalinowski Deli/847-259-7205) and at Golf Plaza II (1016 S. Elmhurst Rd./847-981-9360.) Appointments are required.

Free Tax Prep for active duty military and their families: The Illinois CPA Society offers free tax preparation to active duty military members and their families throughout the state as part of the Military Service Tax Preparation Project. For more information, call 1-800-993-0407.
